Publish retries no longer get wedged

Fixes a publish-path bug where failed uploads could leave tasks stuck pending, blocking retries until manual cleanup.

Failed publishes now transition out of pending (crowlKatsed7c94a)

The publish handler now explicitly marks a task as failed if tarball upload or enqueueing fails, instead of leaving it in pending forever. That unblocks retries for the same version and prevents the CLI from getting stuck polling a task no worker will ever pick up.

Other misc changes

  • Stale-task reaper now considers pending publishing tasks too.
  • Publish flow refactors and related DB/query/test updates to support the retry fix.
