Clarify realm-sensitive prototype fallback (

The spec now says the fallback prototype comes from the constructor’s realm via [[Intrinsics]], and GetPrototypeFromConstructor returns early when prototype is already an object. This is an editorial clarification, but it makes the realm dependency explicit and removes the extra note in favor of direct algorithm text.
